Required Forms for 2023-2024
Please note that the NYS Physical for all school aged students has changed beginning the 2023-2024 school year. In order for students to participate in physical education or any sports program this form must be completed. The new requirement includes the doctor checking the box that states they have reviewed the student family cardiac history. This has been added to asses the risk of sudden cardiac arrest of students.
